| How to Request a Special Recognition
You may request a greeting by postal mail, fax, or email using the contact information below. In your request, please specify the following:
- type of greeting you would like (letter, citation, resolution, proclamation)
- the date of the event
- the name and prefix (Mr., Mrs., Ms., etc.) of the recipient
- the mailing address to which you would like the greeting sent or if you would like to invite
Representative Naughton to present it
- any specific instructions (i.e. "Please mail flat for framing")
Special Notes:
- For birthday requests, please include the recipient's age.
- For anniversary requests, please include the names of both spouses and the number of years married.
- To ensure that you receive your greeting on time, please be sure to submit your request at least three weeks prior to the event being recognized.
